Yampa Valley residents asked for input on community health needs

Friday is the last day for community members to enter survey input for the Yampa Valley Community Health Needs Assessment. The survey is conducted every three years to help identify key health concerns facing residents in Routt and Moffat counties.

Residents 18 and older who complete the 2025 Community Health Needs Assessment survey online can register for a chance to win a $250 gift card. The survey is available at: TinyUrl.com/msbtmuv4. It can be taken in English or Spanish by using the button in the top right corner of the webpage to select the preferred language.

A Community Health Needs Assessment is used by public health organizations, hospitals and community stakeholders to evaluate and prioritize the health needs of a specific population. The process helps to focus on areas needing intervention and to inform policy and funding decisions for community health programs. 



The assessment is presented by Northwest Colorado Health, UCHealth Yampa Valley Medical Center, Memorial Regional Health, The Health Partnership and the Routt and Moffat county public health departments.

In addition to the communitywide survey, partner agencies are hosting two community focus groups this week: from 6-8 p.m. Wednesday in Hayden, the meeting will be facilitated in English; and 6-8 p.m. Friday in Steamboat Springs, the meeting will be facilitated in Spanish.



The focus group on Wednesday in English will be held at the Hayden Center, 495 W. Jefferson Ave. near downtown Hayden. The Friday focus group in Spanish will he held at Library Hall, 1289 Lincoln Ave. in Steamboat.

Participation in either of the focus groups includes dinner and a $25 gift card.
