Health needs assessment survey closes Tuesday, Feb. 22

The 2022 Health Needs Assessment survey for Routt and Moffat counties — a communitywide survey designed to identify health needs and experiences in the Yampa valley — will close on Tuesday, Feb. 22.

The survey is a partnership between a variety of local entities such as The Health Partnership, UCHealth, Northwest Colorado Health, Routt County Public Health, Memorial Regional Health and Routt County United Way.

The survey takes about 10 minutes to complete and can by found at TheHealthPartnership.org/chna. The hope is the survey can reach more than 1,000 people.



The data will be useful in particular to the Routt County Public Health Department because they are currently working update the overall community assessment that will help shape department priorities.

“That’s data that we’re going to put on a dashboard and have that more visible to the public as well,” said Roberta Smith, county public health director.
